This nurse will be responsible for coordinating palliative care of patients in Home Care and advising/assisting those co-workers in identifying patients that need to be transitioned to Hospice. This nurse may also case manage some patients from the transition period throughout their Hospice length of stay.

Mercy Hospital Jefferson, 1400 US Highway 61, Festus, Missouri 63028
Hours/
Schedule:
Full-Time
Days:
Monday - Friday. No Weekends or Holidays.
- Education: Graduate from a RN program.
- Licensure: Current RN license.
- Experience: Two or more years related experience.
- Certifications: BLS certified through American Heart Association.
- Other: Current state driver's license and auto insurance may be required based on the position/unit hired to.
