LPN - Home Health Jefferson

Overview

Mercy – Home Health LPN (Community-Based) opportunities available in Mercy Jefferson Metro Area and neighboring counties. Provide skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Assess, plan, implement, and evaluate patients  needs and care.

• Manage a home health caseload of patients with a wide range of health conditions.
• Deliver patient education, treatment, and emotional support to help patients recover from injuries and illnesses and/or manage chronic disease processes.
• Demonstrate excellent assessment skills and technical proficiency; anticipate and respond to changes in clinical status.
• Establish and maintain excellent rapport, understanding, and communication with patients and family members.
• Communicate with physicians and other healthcare professionals regarding home health patients in a consistent and professional manner.
• Prepare concise and accurate documentation related to changes to medications and plan of care.

• Licensure:
  Graduate of an accredited practical nursing program. Must obtain and maintain a current LPN license in the state where nursing duties are performed; meet all State Board of Nursing requirements.
• Experience:
  
  1 year of experience.
• Certification(s): CPR (Cardiopulmonary Resuscitation).
• LPN-IV Certification must be completed or successfully completed within 3 months of hire for LPNs in Missouri.
• Must be a licensed driver.

• Full-Time, Monday–Friday, 8:00 AM–4:30 PM
• Rotating On-Call Weekends
• Holiday Rotation: 1 summer holiday and 1 winter holiday per year
