LPN - Home Health Jefferson

Responsible for providing skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Must assess, plan, implement, and evaluate patients' needs and care.

A home health nurse manages a caseload of patients experiencing a wide range of health conditions. The nurse delivers pertinent patient education, treatment, and emotional support to aid recovery or chronic disease management. Excellent assessment skills and technical proficiency are required, along with the ability to anticipate and respond to changes in a patient’s clinical status.

Responsibilities include building rapport, understanding, and communicating with patients and their families. The nurse communicates with doctors and other health professionals consistently and professionally, providing concise and accurate documentation of medication changes and plan of care updates.

  Full‑time, Monday–Friday, 8:00 AM–4:30 PM.
• Rotating On‑Call Weekends.
• Holiday Rotation:
  One summer holiday and one winter holiday per year.
• Mileage Reimbursement.

• Licensure
  :
  Graduate of an accredited practical nursing program, or comparable education and experience with successful licensure. Must maintain a current LPN license in the state of service and meet all State Board of Nursing requirements. Must be a licensed driver.
• Experience
  :
  Minimum 1 year of experience.
• Certifications
  : CPR (Cardiopulmonary Resuscitation). LPN‑IV Certification completed or achieved within 3 months of hire for Missouri LPNs.
