Primary Operator

As a Primary Care Nurse at Mercy in our Healthcare & Medical Services department, you'll deliver compassionate, patient-centered care across diverse populations. Responsibilities include comprehensive assessments, chronic disease management, medication administration, and coordination of individualized care plans. You'll collaborate with physicians and interdisciplinary teams, utilize EHR for accurate documentation, and provide education that empowers patients and families. • Perform comprehensive patient assessments and monitor vital signs
• Administer medications and treatments per provider orders
• Develop, implement, and evaluate individualized care plans
• Manage and support patients with chronic conditions (e.g., diabetes, hypertension)
• Document care accurately in the electronic health record (EHR)
• Educate patients and families on disease management and preventive care
• Coordinate care and referrals with physicians and interdisciplinary teams
• Maintain infection control and safety standards
• Participate in quality improvement and evidence-based practice initiatives
