RN – Home Health Jefferson

Seeking a skilled and compassionate RN to provide home health nursing services under physician supervision.

Responsibilities include:

• Assessing, planning, implementing, and documenting patient care outcomes.
• Managing a caseload of patients with diverse health conditions in their homes.
• Providing patient education, treatment, and emotional support to promote recovery and manage chronic illnesses.
• Responding to clinical changes with excellent assessment skills and technical proficiency.
• Communicating effectively with patients, families, and healthcare professionals regarding care plans and medication updates.

• Current RN license in Missouri.
• Preferably one year of experience in home health or related fields.
• Graduation from an accredited nursing program.
• Valid driver’s license, auto insurance, and BLS certification through the American Heart Association.

• Excellent assessment, communication, and documentation skills.
• Ability to build rapport and understand patient needs.
• Independent, compassionate, and detail-oriented.
• Provides high-quality, patient-centered care in a community setting.
• Offers flexible schedules, mileage reimbursement, and opportunities to serve diverse patient populations.
