Overview
NHS Fife Pharmacy and Medicines Directorate are seeking an individual to join our dynamic and passionate Community Frailty and Hospital at Home Pharmacy Team, dedicated to delivering exceptional care to ensure safe, timely, and efficient medicines management and supply for patients receiving care will be working at ward/clinic level with the Community Hospital Pharmacy Team and the wider Pharmacy service. You’ll play a pivotal role in enhancing patient experience, facilitating communication across care providers, assisting in taking medications histories, managing medications supply, maintaining accurate records and ensuring the utmost safety and effectiveness in their hospital journey.

- Work at ward/clinic level with the Community Hospital Pharmacy Team and the wider Pharmacy service.
- Assist in taking medications histories and managing medications supply.
- Maintain accurate records and ensure safety and effectiveness in patients hospital journeys.
- Enhance patient experience and facilitate communication across care providers.
- Experience working in a pharmacy team.
- SVQ in Pharmacy Services at SCQF Level 6 (Full award) or equivalent.
- Have completed or be willing to work towards the underpinning knowledge programme for Pharmacy Support Workers at Fife College.
- Good interpersonal and communication skills; a passion for patient care.
- Forward thinking, flexible, and able to enjoy face-to-face interactions with patients and the multidisciplinary team.
- Ability to travel throughout Fife.
The NHS Fife Pharmacy and Medicines Directorate provides an integrated service to a population of approximately 380,000 people, with a team of around 300 Pharmacy staff, including Pharmacists, Pharmacy Technicians, Support Workers, Nurses, and Administrators. Teamand working environment
The team works across Acute and Community hospital sites, General Practices, Mental Health services, and other specialist teams. Partnership working is core to our values and we collaborate with multidisciplinary colleagues, including Community Pharmacy, to deliver high-quality care across Fife. Fife offers good transport links and proximity to major cities.

- From 1/4/26, the Agenda for Change full-time working week will reduce from 37 to 36 hours; part-time hours will be pro-rata with a corresponding increase in hourly rate.
