The position will collaborate with the Person-Centered Practices Specialist to ensure knowledgeable and use of person-centered planning, and how it applies to supporting this specific group of people and families.
This position will collaborate with the Benefits Specialist and the Multi-Cultural Specialist to ensure they are using innovative approaches to service coordination and accessing generic resources.
Specific

Job Duties

• Manage a 1:40 caseload ratio currently served by TCRC with the low POS and/or no POS expenditures.
• Utilizing a Person-Centered approach to the Individual Program Plan Process, will work to identify barriers is accessing services and supports; to enhance relationship between individual/family and TCRC; to empower individuals and families to partner with TCRC and other community agencies and to assist in identification of new resources that are culturally relevant to their community.
• Works in collaboration with the PCP Specialist, Cultural Specialist, Self-Directed Services team and other TCRC Departments to identify innovative approaches to service coordination.
• Using PCP strategies will be responsible for the development and implementation of the individual’s IPP, progress reports, annual reviews, and quarterly in person reviews.
• Provides follow-up, reassessments, and periodic review of individual’s progress and program effectiveness.
• Adheres to the agency mission, philosophy and core values.
• Assists in locating and developing natural supports.
• Initiates the purchase of vendored and generic services and closely monitors service delivery. As well as assists in navigating state-wide systems such as Departments of Managed Health Care, Social Services and Rehabilitation.
• Keeps abreast of state and federal regulations, agency policy and procedure and guidelines related to services for individuals. Acts as an advocate on social, legal, education, mental health and/or forensics issues related to the individual.
• Participates in outreach and represents the agency in community meetings and events.
• May conduct public presentations regarding regional center services and activities.
• May serve on intra-agency and/or inter-agency committees.
• May participate in data gathering and analysis activities.
• Additional duties may be assigned as deemed appropriate(i.e. community presentations, attending local resource fairs, etc.

• Conduct strength-based assessments that communicate an understanding of the unique issues facing family members.
• Involve individuals we serve and family in all aspects of person-centered planning and support activities.
• Design, document, implement and monitor services and supports in a timely manner.
• Routinely contact individuals and families to answer emerging questions, quarterly in-person review meetings and annual IPP or IFSP meetings.
• Facilitate planning team progress, identifying outcomes, needed services and supports. Ongoing data collection.
• Develop and maintain a network of vendors and community agencies.
• Provide relevant information and training about developmental disabilities, services and resources.
• Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families of varying cultural backgrounds.
• Demonstrate an operative knowledge of family/natural support and private/public resources and integrate these into service planning resulting in community-based outcomes.
• Demonstrate an operative knowledge about developmental disabilities and how families are impacted.
• Demonstrate an operative knowledge of a variety of approaches to intervention and support.
• Demonstrate an operative knowledge of entitlement and benefit programs.
• Demonstrate an operative knowledge of the Tri-Counties Regional Center system, the legal system, individual civil rights and overall advocacy.
• Work in a cooperative and collaborative manner as a team member.
• Develop and maintain good relationships with community representatives.
• Participate in professional growth and development through attendance at in-service and other training activities, conferences and other job-related agency approved events.
• Conduct all activities in a professional and ethical manner.
