North Hospice Team.
Responsibilities- Works closely with RN Case Managers to meet the needs of hospice patients and their families in home and long‑term care facilities.
- Demonstrates performance consistent with professional standards of practice, care, performance, the Nurse Practice Act, and the mission and goals of the organization.
- Manages patient care in the health‑care continuum to achieve optimum outcomes in a safe and cost‑effective manner.
- Graduate of a recognized accredited school of practical nursing, or graduate from a non‑accredited school of practical nursing with three years related professional nursing experience.
- Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency.
- Valid Driver's License.
