Make a Difference — Community Hospice is seeking an engaging Patient Care Coordinator to lead and support exceptional clinical care for our hospice patients and their loved ones. This leader helps the team make a difference one life at a time through individualized care that promotes comfort and dignity. The position requires working closely with the Director of Nursing and providing support in day‑to‑day operations of the Hospice program, including supervision of direct care workers in the patient homes and from the office.

As the Patient Care Coordinator, you will exhibit excellence in clinical patient care, customer service, critical thinking, schedule management, relationship competencies, and organizational skills. In leading this meaningful work, you will be able to focus on patient needs and have the authority and autonomy to ensure appropriate clinical and customer satisfaction outcomes are met, while effectively leading your team.

This position works Monday – Friday, serving in management with an on‑call rotation 1‑2 times per month to support after‑hours, weekend and holiday patient care. It is a full‑time, benefits‑eligible position.

Exceptional

Skills and Qualifications
  • Graduate of a National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Bachelor’s degree preferred.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana listed in the Nurse Licensure Compact (NLC).
  • Two (2) or more years of nursing experience, Hospice preferred.
