RN Case Manager - Pediatric Home Health
We’re hiring.
Location: Worcester County, Massachusetts
Schedule: Monday–Friday, daytime hours (primarily field‑based)
Pay: $90,000–$105,000 base salary + $2,000 sign‑on bonus
What You’ll Do (Day‑to‑Day)- Manage a pediatric home health caseload
- Complete 2–5 home visits per day, typically 45–60 minutes each
- Assess patient progress and ensure care plans are followed
- Coach, train, and support family caregivers and home health aides
- Identify gaps, recommend changes, and coordinate updates with the care team
- Complete accurate electronic documentation within 24 hours
- Collaborate with providers and internal clinical leadership to drive outcomes
- Manage a pediatric home health caseload
- Conduct 2–5 home visits per day within assigned territory
- Assess patient progress and ensure care plans are followed
- Train, support, and monitor family caregivers and home health aides
- Evaluate alignment between care needs and authorized service hours
- Identify gaps in care and recommend adjustments
- Monitor clinical and developmental progress in home and school settings
- Complete timely, accurate documentation (within 24 hours)
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Allocate 1 day per week for administrative/documentation time
- Hybrid role (field visits + administrative time)
- Primarily Monday–Friday schedule
- Standard daytime hours with some flexibility
- Caseload‑based workflow
- Active Massachusetts RN license
- 2+ years RN experience (no new grads)
- 1+ year home health or home‑based care experience
- 1+ year pediatric experience (home‑health pediatrics preferred; pediatric acute care considered)
- Reliable vehicle, valid driver’s license, and auto insurance
- Familiarity with OASIS
- Experience with Medicaid‑funded community/home or family‑caregiving models
- Experience with electronic documentation systems
- Strong communication skills with families and interdisciplinary teams
