to fill that mission.

Salary:$22.56-$25.95 Hourly

Schedule: Monday - Friday 8:00AM-4:00 PM (40 hr/week)
* ability to be flexible with schedule is highly preferred*

Essential Job Functions:

  • The Person-Centered Plan Manager, also known as a case manager, works directly with the individual in the community to help them achieve their goals. Typically, approximately 70% of time is spent in the community.
  • Connects the individual with the services they need to maintain and expand their independence. This may include community services, government-funded benefits, or other publicly available resources.
  • Advocates for the individual ensuring that they receive the proper support for their situation.
  • Completes administrative tasks on behalf of the individual, including consultations, documentation into an electronic record system, benefits eligible forms, and other related tasks.

Qualifications:

  • Associate degree or equivalent experience, preferably in a related field.
  • Valid driver's license with an acceptable driving record and ability to pass background checks.
  • Basic computer skills, including Microsoft Office, email communication, internet, and documentation entry.
  • Ability to work independently and collaboratively as part of a team.
  • Compassion, resourceful, and a strong desire to learn and grow.
