Nurse Practitioner​/Physician Assistant – Outpatient Dialysis

• Participate in multidisciplinary hemodialysis rounds
• Collaborate with the patient’s care team for care coordination
• Write and manage dialysis orders
• Manage and treat anemia and bone mineral disease
• Assess and refer for dialysis access placement or intervention
• Manage blood pressure
• Assess volume status and estimated dry weight
• Review and manage lab results
• Refer patients for transplant and/or home dialysis programs
• Work at the Mile Bluff, Lake Delton, and Portage clinics

• Nurse Practitioner or Physician Assistant qualification
• Nurse Practitioner:
  Master’s or Doctorate’s degree from an accredited Nursing Program
• Physician Assistant:
  Bachelor’s or Master’s degree from an accredited PA Program
• State of Wisconsin NP or PA licensure
• Nurse Practitioner:
  Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice
• Nurse Practitioner:
  Active board certification as an NP in the area of clinical practice:
  Adult;
  Adult/Gerontology Primary Care; or Adult/Gerontology Acute Care
• Active DEA license or ability to apply for such license prior to or upon hire

Demonstrates expertise in managing hemodialysis care, including writing dialysis orders, managing anemia, and coordinating with multidisciplinary teams. Holds necessary qualifications and licensure to provide comprehensive patient care in a clinical setting.
