Registered Nurse Case Manager
We are seeking a Registered Nurse Case Manager to oversee hospice patient care, develop and implement personalized Plans of Care, and evaluate patient needs regularly.
Responsibilities- conducting initial assessments within 48 hours of referral
- identifying physical, psychosocial, and environmental needs
- coordinating care plans
- documenting patient responses
- collaborating with patients, families, physicians, and the interdisciplinary team
current RN license, at least one year of clinical experience, CPR certification, a valid driver’s license, and reliable transportation.
Preferred Skillsindependence, strong communication, organizational abilities, and effective written skills.
