Coconino County is seeking a Reentry Coordinator for Pathways to Community, a collaboration between the Coconino County Sheriff’s Office and Coconino County Health and Human Services (CCHHS) that supports reentry for those leaving jail.
The role coordinates comprehensive case management and navigation services for incarcerated or recently released individuals, working with on-site partners to connect housing, social, behavioral, transportation and legal services for a successful transition.
