Telehealth Nurse - Home Health

Position Summary

Provides, arranges for, teaches and/or supervises nursing care through the use of telehealth patient encounters, telephonic follow-up calls, and patient visits. Routine weekend and holiday telehealth monitoring; PTO coverage for the Telehealth Supervisor as needed. Assists with TH program marketing.

Primary Position Responsibilities

Provides/coordinates patient centered care delivery to patients receiving telehealth home monitoring.

Promotes the successful adoption of patient disease self-management behaviors.

Program Promotion.

Other duties as assigned.

Qualifications

Minimum Education:

Required:

All hires after July 2012 are required to have a Bachelor's Degree in Nursing. OR if Associates Degree or Diploma, RN must provide proof of enrollment in a BSN program prior to hire and complete program within five years of hire date.

Minimum Years of Experience (Amount, Type and Variation):

Required:

Two years of Med-Surg experience

Preferred:

Two years of current home care experience. One to two years of management of high risk patients (cardiac, pulmonary, diabetic)

License, Registry or Certification:

Required:

Current CPR certification, NJ RN licensure, valid driver's license

Preferred:

Certification in specialty

Knowledge, Skills and/or Abilities:

Required:

• Excellent physical assessment skills.
• Excellent communication skills.
• Familiar with Microsoft Office & Google Suite.

Preferred:

Knowledge of state and federal home health regulations.
