Hospice Nurse
Provides continuity of hospice care to Hunterdon Hospice patients and families through the application of the nursing process using evidence based practice focusing on excellent outcomes and patient experience to maintain and maximize the comfort of both patient and family. Ensures quality and continuity of care in the absence of the primary nurse.
Primary
Position Responsibilities:
- Ensures continuity of nursing care under medical direction in accordance with recognized standards of care, hospice philosophy and established policies of Hunterdon Hospice. This may take place during the hours the office of Hunterdon Hospice is closed
- Assesses problems and promotes resolution of emergency situations when the office of Hunterdon Hospice is closed
- Admits patients to Hunterdon Hospice services and hands off to primary nurse
- Actively participates in Hunterdon Hospice Quality Improvement
Qualifications:
Minimum Education:
Required:
Bachelor’s Degree in nursing OR if Associates Degree or Diploma, RN must provide proof of enrollment in a BSN program prior to hire, be continuously enrolled and complete within 5 years of hire date or internal RN promotion date. Associates Degree AND 2 years of experience in medical/surgical, oncology, hospice, palliative or community health nursing will be accepted in lieu of BSN.
Minimum Years of Experience (Amount, Type and Variation):

Two years experience in medical/surgical, oncology, hospice, palliative or community health nursing.
License, Registry or Certification:
Required:
New Jersey License as Professional Registered Nurse. Preferred:
Certified Hospice and Palliative Care Nursing (CHPN).
Knowledge, Skills and/or Abilities:
Required:
Strong interpersonal and communication skills, strong organizational skills, strong assessment skills. Preferred:
General computer experience, IV certification / experience, knowledge of palliative and hospice care, knowledge of hospice conditions of participation and federal regulation.
