In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. This is an opportunity to contribute meaningful social work expertise in a care setting where emotional, spiritual, and practical support are central to the patient and family experience.

Responsibilities
  • Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans.
  • Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
  • Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record.
  • Conduct reassessments of patient and family needs and provide counseling as required.
  • Provide crisis intervention and individual or family counseling when indicated.
  • Serve as a care advocate for the patient and family unit.
  • Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.
  • Provide ongoing counseling related to issues of death and dying to the patient and family as needed.
  • Participate as a member of the Bereavement Team as assigned.
  • Adhere to all hospice policies.
