Oak Street Health is seeking a Medical Social Worker to support psychosocial needs of our highest-risk patients. You will assess, advocate, and coordinate care with an interdisciplinary team to improve health outcomes and reduce hospitalizations.
The role requires an MSW, state licensure, and proficiency with EMR systems. Fluency in Spanish/Polish/Russian or other languages is preferred. 40 hours weekly, with a comprehensive benefits package.
