We are seeking a compassionate and experienced Physical Therapist to join our team at Briarwood Nursing and Rehab in Flint, Michigan. As a Physical Therapist, you will be responsible for designing and implementing individualized treatment plans to help patients achieve their rehabilitation goals. You will work closely with our interdisciplinary team of healthcare professionals to provide high-quality patient care and education. Your expertise will be instrumental in helping patients regain independence, manage chronic conditions, and achieve optimal functional outcomes.

• Conduct initial evaluations and assessments to identify patient needs and develop treatment plans
• Implement evidence-based therapies, including physical therapy exercises, modalities, and education
• Monitor patient progress and adjust treatment plans as needed
• Participate in interdisciplinary team meetings to collaborate with other healthcare professionals
• Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
• Maintain accurate and thorough patient records and documentation
