Make a meaningful difference where patients are most comfortable...nson at Home is seeking a compassionate and skilled Occupational Therapist to join our Home Care team on a PRN basis. In this role, you'll help patients regain independence, improve daily function, and achieve their highest quality of life through personalized, in-home therapy services. WhatYou'll Do
- Evaluate patients' functional abilities and develop individualized treatment plans
- Deliver evidence-based occupational therapy services in the home setting
- Help patients improve their ability to perform daily living activities safely and independently
- Educate patients, caregivers, and family members on treatment strategies and adaptive techniques
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care
- Monitor patient progress and modify treatment plans as needed
- Recommend and train patients on adaptive equipment, splints, and assistive devices when appropriate
- Maintain accurate and timely electronic documentation
- Participate in interdisciplinary care planning and process improvement initiatives
- Serve as a clinical resource and mentor to students and support staff as needed
- Bachelor's degree in Occupational Therapy
- Current Occupational Therapist license in good standing in the State of Michigan
- Current Michigan driver's license
- Reliable transportation available for daily business use
- Proof of current automobile insurance
- Ability to obtain BLS certification within three months of hire
- Strong communication and interpersonal skills
- Basic computer and mobile device proficiency
- Previous clinical experience in occupational therapy
- Home care experience
- One-on-one patient care in the home environment

This position requires frequent standing, walking, bending, reaching, and patient assistance activities. Candidates must be able to lift, move, push, or pull up to 50 pounds and occasionally assist with heavier physical demands associated with patient care.
