This position may be full-time or part-time/per diem and involves in-home evaluations and therapy for patients, with supervision of a COTA and collaboration with a clinical team.
You’ll deliver evidence-based treatments, assess safety and independence at home, document progress in Well Sky/Kinnser EMR, and travel to patient homes in Alameda/Contra Costa
