Outpatient Case Manager - Mental Health

Macomb County Community Mental Health is seeking a Case Manager
- Outpatient North to deliver direct clinical services to adults and families in a community mental health setting.

You will coordinate with health care providers, document treatment plans, and participate in team meetings to ensure patient-centered care. A Bachelor’s degree in a mental health field and QMHP qualifications are required; a Master’s degree is preferred.
