Full Time Registered Nurse – Genesee County

Job DescriptionOptimal Care is seeking a Full Time Registered Nurse committed to providing the highest quality of care. We are looking for highly professional, self‑motivated RNs who love working with the adult and geriatric homebound population. Home care experience is preferred but not required.
We know you work diligently every day, caring for your patients. Our company is owned by nurses, so we understand the expectations and challenges of being a Home Health Nurse.
Key Responsibilities- Provide services requiring substantial and specialized nursing skill, in accordance with the plan of treatments signed by the physician, and may make the initial evaluation visit to the client.
- Provide initial and ongoing comprehensive assessments of the client's needs, including OASIS assessments at appropriate time points, and re‑evaluate nursing needs; initiate, develop, implement, and revise the care plan.
- Initiate diagnostic, preventative, and rehabilitative nursing procedures as appropriate to the client's care and safety; make referrals to other disciplines as indicated.
- Observe signs and symptoms and report to the physician or appropriate health professional as needed.
- Teach, supervise, and counsel the client and family regarding home nursing procedures and other care needs.
- Coordinate the total plan of care and maintain continuity of care by liaising with other health professionals; attend client care conferences and initiate conferences for complex clients.
- Assign and supervise home health aide services, align with Medicare Conditions of Participation, and assess need for unskilled care; revise the plan as appropriate.
- Supervise LPNs and paraprofessionals providing services.
- Develop and maintain individualized client care progress records with accuracy and timeliness.
- Follow all policies and procedures regarding service delivery, documentation, and care coordination with full compliance.
- Participate in in‑service programs and quality improvement initiatives.
- Plan for client discharge from services.
- Minimum associate's degree in nursing from an accredited program.
- Registered Nurse license recognized by the state in which you practice.
- Minimum of 1 year experience as a registered nurse preferred.
- Knowledge and skills to provide care to and communicate with adult and geriatric populations.
- Knowledge of principles of growth and development across the lifespan.
- Ability to assess client data and interpret information to identify client‑specific needs.
- Competent computer skills for using the electronic medical record.
- Valid driver's license and current auto insurance.
- Reliable transportation.
$75,000 - $85,000 USD
