Responsibilities
What You'll Do
As our Start-of-Care RN, you'll be the one who kicks off each patient's journey with us.
Your role is vital, meaningful, and full of impact. You will:
• Complete start-of-care assessments with accuracy, compassion, and confidence
• Build trust with patients and families from the very first visit
• Develop individualized care plans that set patients up for success
• Coordinate with physicians, therapists, and the clinical team to ensure seamless transitions
• Document efficiently using our supportive EMR system
• Serve as a clinical resource and role model for the team
• Bring positivity, professionalism, and a can-do attitude to every day
This role is perfect for a nurse who thrives on autonomy, loves meeting new people, and enjoys being the "first face" of exceptional care.
Requirements
What You Bring
• Active RN license
• Strong clinical assessment skills
• Home health or start-of-care experience preferred, but we'll train the right superstar
• Excellent communication and relationship-building abilities
• Comfort with electronic documentation
• A reliable vehicle and a passion for serving your community
• A ROCK STAR mindset - confident, compassionate, and ready to shine
