Overview
Summary The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse for the Patient Aligned Care Teams (PACT) at the Flint VA Community Based Outpatient Clinic (CBOC) - within Ambulatory Care Service. The tour for this position is:
Monday-Friday 7:00am-3:30pm. EDRP eligible.

Basic Requirements:
- Citizenship:
Citizen of the United States - Education:
Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing (NLNAC) Accrediting Commission at the time the program was completed - Verification can be obtained from the State approving agency or the National League for Nursing Accrediting Commission

- Exceptions include:
Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure - Individuals granted a license by a jurisdiction that does not require graduation from an approved school
- Licensure:
Full - active - current and unrestricted licensure as a licensed practical or vocational nurse in a State/Territory/Commonwealth or District of Columbia - Licensing history must comply with VA requirements (no revoked/suspended licenses unless allowed by policy)
- English Language Proficiency:
Proficient in spoken and written English as required by policy - Grade Determinations: GS-3 to GS-6 as described in VA Qualification Standard (GS-3: basic requirements; GS-4/5/6 criteria apply as experience increases)
- Perform assigned nursing care and document actions taken for commonly prescribed medications and treatments
- Administer medications and monitor patient responses in accordance with facility policy and procedure
- Assist with patient assessments, monitoring, and coordination with the RN/MD/DO
- Provide education to patients and families on disease processes, medications, and prescribed treatment plans
- Collaborate with the nursing team to promote quality health care and positive patient experiences
- Prepare and document patient care activities, including vital signs, treatments, and outcomes
- Recognize emergencies and initiate appropriate interventions, seeking assistance as needed
- Identify equipment issues and safety hazards and report for corrective action
- Works under the supervision of an RN or MD/DO with broader scope as competency develops
- Maintain general knowledge of practical nursing procedures and adherence to policies
- Maintain patient privacy and dignity during care
- Team-based patient care within PACT; VHA standards and customer service expectations
- Direct patient care including rooming patients, obtaining vital signs, and performing treatments as ordered
- Phlebotomy, immunizations, and clinic-specific duties (e.g., foot/toenail clinic, quality metrics, and population health tracking)
- Assessment, data collection, and documentation in nursing notes; patient education and health coaching aligned with patient goals
- Medication reconciliation and administration (oral, topical, subcutaneous, intramuscular, IV as allowed by policy)
- Emergency preparedness, crash cart checks, and adherence to safety protocols
- EKG, laboratory draws, pre-processing, and point-of-care testing as assigned
- Comprehensive duties across multiple clinics as described in the job materials
