Summary
The LTC Charles S. Kettles VA Medical Center is seeking a full‑time Licensed Practical Nurse for the Patient Aligned Care Team (PACT). The LPN is aligned under Ambulatory Care Services at the Flint Community‑Based Outpatient Clinic (CBOC).
Qualifications- United States Citizenship:
Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. - English Language Proficiency. In accordance with 38 U.S.C. 7402(d) and 7407(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- Education – Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions include health‑care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
- Current GS‑0620 LPN/LVN employee of the Veterans Health Administration (VHA) who was converted to an appointment under 38 U.S.C. 7401(3) in implementation of Public Law 98‑160 enacted November 21 - 1983 or who was converted to an appointment under 38 U.S.C. 7405 in implementation of Public Law 99‑576 enacted October 28 - 1986.
- Individual granted a license by a jurisdiction that does not require graduation from an approved school.
- Licensure – Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e. Puerto Rico) – of the United States or the District of Columbia.
- Grade determinations and competency requirements for GS‑03 through GS‑06 as outlined in the posting.
- The primary purpose of the Licensed Practical/License Vocational Nurse PACT (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran.
- Serves as a member of the health‑care team by participating in planning, implementation and evaluation of nursing care using a focused assessment; functions as preceptor when appropriate; administers medications via all routes excluding intravenous; educates patients and/or family for understanding of orders as documented in the EHR; performs assigned nursing care responsibilities under limited to moderate supervision, including use of certain equipment, technology and/or knowledge in procedures performed in clinics.
- Documents obtained vital signs (pulse, blood pressure, respirations, temperature and pain level); completes clinical reminders for preventive health screening including mental health screening.
- Recognizes and documents urgent and emergent patient care situations and seeks assistance of the RN and/or provider.
- Takes appropriate actions for positive reminders; laboratory responsibilities (site specific) include blood draw, proper handling of specimens, use of centrifuge, ordering and organizing of lab supplies, daily monitoring of View Point and proper packaging of samples for testing.
- Performs diabetic foot screening, including visual screening, pulses and sensation.
- Refers patients to appropriate resources including social work, dietitian, speech pathology, smoking cessation and physical and occupational therapy after screening.
- Recognizes and successfully cares for patients of different ages – including young adults, middle‑aged adults and older adults – demonstrating understanding of how age affects the health‑care needs of patients.
- Participates in notifying patients of test results; reviews consults for designated clinics using established protocols; refers more complex consults to Care Manager or provider in clinic.
- Provides input in clinic process improvements and systems redesign activities; actively supports clinic in meeting and exceeding performance measure goals.
- Applies knowledge of TJC accreditation standards, National Performance Goals and Infection Control to everyday practice.
