LPN/LVN, Private Duty Nurse
Become a part of our caring community as a Center Well Home Health nurse. In this role you will provide personalized, one‑on‑one care to patients in their homes, manage care plans, and support patients as they regain independence with the backing of a dedicated medical team.
Responsibilities- Provide skilled nursing care to patients onsite, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self‑care techniques.
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other healthcare team members promptly regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Experience in a Home Health or Hospice setting.
- Pay per visit/unit: $33.00 – $47.00.
- Base pay: $53,000 – $73,000 per year.
- Contracted weekly hours: 40 hours.
- TB Statement:
This role is patient–facing and is part of Humana’s Tuberculosis (TB) screening program. Screening for TB is required if selected. - Driving Statement:
This role is part of Humana’s driver safety program. A valid state driver’s license is required, and the employee must maintain personal vehicle liability insurance meeting state minimum limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage, whichever is higher.
