Job Summary The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders and the patient’s plan of care. The RN ensures high-quality, patient-centered care while collaborating with the interdisciplinary team to promote health, safety, and independence.
Responsibilities Provide skilled nursing care to patients in accordance with the plan of care Perform comprehensive patient assessments and ongoing evaluations
Administer medications and treatments as prescribed, following safety protocols
Collaborate with physicians and interdisciplinary team members to develop and update individualized care plans
Monitor patient conditions and report changes promptly
Perform skilled nursing procedures, including wound care, IV therapy, catheter care, and related treatments
Maintain accurate, timely clinical documentation in compliance with HIPAA and agency standards
Educate patients and families on disease processes, medications, and home care needs
Participate in case management and care coordination to support successful outcomes PROFESSIONAL EXPERIENCE/
QUALIFICATIONS:
Maintains the agency’s mission, philosophy and core values.
Adheres to dress code, appearance is neat and clean, wears identification while on duty.
Reports to work on time and/or as scheduled, notifies patients for delays in expected arrival times.
Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
Always maintains patient confidentiality, including all HIPAA regulations.
Complies with agency “On-Call” requirements as specified by the Clinical Manager or Administrator.
REGULATORY REQUIREMENTS Licensed or registered in the worked in state.
Must pass a criminal background check & MVR check.
Completed health statement acknowledging ability to perform the duties of the position.
Valid state driver’s license.
Proof of current automobile insurance.
Current CPR cardTB testing per agency policy
