Job Overview

As a Center Well Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care to help patients regain independence in the comfort of their homes.

• Provide skilled nursing care to patients at their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family and healthcare team.
• Educate patients and families on disease management, treatment options and self‑care techniques.
• Maintain accurate records of patient care and documentation of visits, interventions and outcomes.
• Communicate promptly with patients, physicians, the clinical manager and other team members regarding patient or family needs and status changes; participate in care coordination and discharge planning.

• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

• Experience in a Home Health or Hospice setting.

• Pay per visit: $33.00 – $47.00.
• Base salary: $53,000 – $73,000 per year.
• Scheduled weekly hours: 40.

• TB Statement:
  This role is patient facing and part of Humana’s TB screening program; TB screening is required.
• Driving Statement:
  Requires a valid state driver’s license, personal vehicle liability insurance in line with state minimums.
