In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Marquette, MI

Overview

Signify Health, part of CVS Health, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves engaging with patients in their homes, conducting a 45‑minute visit that includes a medication and medical history review, physical assessment, and, when ordered by a health plan, additional diagnostic tests such as diabetic eye exams or spirometry.

• Use an iPad and an intuitive clinical workflow for each evaluation.
• Not required to prescribe medicine, order lab tests, or alter existing treatment regimens.
• Provide diagnostic and preventive services products (e.g., spirometry, DEE).
• Conduct virtual visits as needed.

• Bring care and empathy into every visit, building trusted relationships with patients.
• Assist in connecting patients to appropriate care and services.
• Collaborate with Signify Health clinicians to fulfill the role’s requirements.
• Maintain flexibility to travel locally and within licensed states.
• Perform all tasks safely in diverse home environments, including those with pets, smoke, or allergens.

• Pay range: $41.85 – $90.13 per hour (depending on experience, education, geography, and other factors)

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
