Hope Network is seeking a compassionate Case Manager to support adults in Genesee County through person-centered, community-based services. In this role, you will assess needs, develop care plans, and coordinate healthcare, behavioral health, housing, and social supports to promote independence and recovery. You'll advocate for clients, navigate community resources, respond to crises, and collaborate with an interdisciplinary team. Responsibilities
- Provide comprehensive case management for adults in Genesee County
- Conduct intake, assessment, and person-centered service planning
- Coordinate healthcare, behavioral health, housing, and community services
- Monitor progress, update care plans, and document services accurately
- Advocate for client rights, access, and community integration
- Collaborate with interdisciplinary teams and community partners
- Provide crisis intervention and resource navigation as needed
- Maintain caseload within agency and regulatory standards
- Educate clients and families on available supports and resources
- Participate in training, supervision, and quality improvement activities
