Vocational Counselor

Sevita seeks a Vocational Counselor to support individuals in achieving meaningful employment and greater independence. Responsibilities

• Provide vocational counseling and career planning for individuals with diverse abilities and health needs
• Conduct assessments to evaluate skills, interests, and employment readiness
• Develop individualized vocational plans aligned with person-centered care goals
• Coordinate job training, coaching, and placement with community employers
• Collaborate with healthcare providers, case managers, and families to support employment success
• Monitor progress, document outcomes, and adjust plans as needed
• Educate individuals on workplace expectations, accommodations, and self-advocacy
• Maintain accurate records and ensure compliance with program and regulatory standards
