Job Type: PRN

Assesses and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

• Interview and assess patients and/or patients’ family, caregivers, and/or legal representatives.
• Determine, prioritize, provide, and/or arrange for needed internal and external services/interventions.
• Participate in case reviews to evaluate case management and progress.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient’s treatment plan.
• Assist with discharge planning and processes.
• Identify appropriate resources (transportation, housing, healthcare, social/spiritual services) and provide referrals as part of the discharge plan.
• Supervise and/or train new staff, students, and interns.
• Provide individual, family, and group therapy.

• Bachelor’s degree required.
• Board Certified Social Worker (LBSW or LCSW) in Alabama within 6 months of hire.
• Demonstrated critical thinking and decisive judgment skills.
• Ability to work in a stressful environment with minimal supervision.
