Licensed Practical Nurse

Glenwood Center - AL, a Genesis Health Care skilled nursing facility in Prattville, seeks a compassionate Licensed Practical Nurse to deliver high-quality, person-centered care for short-term rehab and long-term care residents. The LPN will administer medications and treatments, monitor conditions, support individualized care plans, and document accurately while collaborating with an interdisciplinary team. Responsibilities

• Provide direct nursing care to residents, including medication administration, treatments, and monitoring of vital signs
• Support development and implementation of individualized care plans in collaboration with RNs and interdisciplinary team
• Document assessments, interventions, and resident responses in accordance with facility and regulatory standards
• Assist with admissions, discharges, and transitions of care, ensuring accurate handoffs and education
• Recognize and report changes in resident condition promptly to supervising nurse or provider
• Educate residents and families on care plans, medications, and health management as appropriate
• Promote resident safety, comfort, dignity, and independence in all aspects of daily care
• Collaborate with CNAs and other team members, providing direction and support to ensure high-quality care
• Maintain infection control, safety, and compliance with state, federal, and Genesis Health
• Care policies
• Participate in quality improvement, staff meetings, and ongoing clinical training and development
