Job Summary:

Provide high-quality nursing care in a Long-Term Care or Skilled Nursing Facility setting as an LVN or LPN. Ensure patient safety, comfort, and well-being through competent clinical support and adherence to facility protocols.

Responsibilities:

• Administer medication and treatments accurately according to physicians' orders.
• Monitor patient vital signs and observe for any changes in condition.
• Document patient care activities thoroughly and promptly in medical records.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Assist with patient mobility, hygiene, and daily living activities.
• Maintain compliance with facility policies, safety protocols, and infection control standards.

Qualifications:

• Current valid LVN or LPN license.
• Demonstrate knowledge of long-term care and skilled nursing procedures.
• Ability to accurately assess patient needs and communicate effectively.
• Must have completed the COVID-19 vaccination or hold a valid exemption.
• Strong attention to detail and adherence to safety regulations.
