Job Summary:

We are seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide quality patient care in a long-term care or skilled nursing facility setting. The successful candidate will support clinical operations, ensuring compliance with healthcare standards and promoting patient well-being.

Responsibilities:

• Administer medication and treatments following physician orders and facility protocols.
• Monitor patient conditions, accurately document assessments, and report changes to the healthcare team.
• Assist with patient hygiene, mobility, and daily living activities to ensure comfort and safety.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Maintain compliance with regulatory requirements, including COVID-19 vaccination policies.
• Ensure proper identification procedures and consistently wear photo badges during shifts.

Qualifications:

• Valid LVN or LPN license in good standing.
• Proven experience in long-term care or skilled nursing facility settings preferred.
• Ability to follow detailed healthcare instructions and maintain accurate documentation.
• Excellent communication and interpersonal skills.
• Mandatory COVID-19 vaccination or valid exemption documentation required.
• Ability to stand for extended periods and perform physical tasks associated with patient care.
