Job Summary:

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting as an LVN/LPN. Deliver compassionate, high-quality clinical support to ensure patient comfort, safety, and well-being.

Responsibilities:

• Administer medication and treatments accurately according to care plans.
• Monitor residents’ health status and report changes to the nursing team.
• Assist with daily living activities and provide emotional support to residents.
• Maintain detailed, up-to-date documentation of patient care and observations.
• Adhere to safety protocols and infection control guidelines.
• Collaborate with interdisciplinary team members to develop and implement care plans.

Qualifications:

• Valid LVN or LPN license and documentation of licensure in the state.
• Proven experience in a long-term care or skilled nursing facility.
• Ability to follow detailed instructions and protocols precisely.
• Strong communication and interpersonal skills.
• Competency in vital signs assessment and medication administration.
• Flexibility to adapt to dynamic patient care needs and schedules.
