Position Title: Nurse Practitioner (NP) / Physician Assistant (PA) ? Family Practice
Location: Florence Family Practice | Florence, Arizona
Schedule: Four 10-Hour Days

• Monday, Tuesday, Thursday: Florence
• Friday: Coolidge

Hours: 8:00 AM ? 7:00 PM
Setting: Outpatient Family Medicine
Travel: Required one day per week (to Coolidge clinic)

Compensation & Benefits

• Salary: $110,000 ? $130,000 annually (based on experience)

Position Overview

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our community-based family medicine clinics in Florence and Coolidge, AZ
. This full-time role provides a consistent, flexible four-day schedule and the opportunity to deliver full-spectrum primary care to patients of all ages in a supportive, mission-driven practice.

Key Responsibilities

• Perform comprehensive physical exams and health assessments
• Diagnose and manage acute illnesses and chronic conditions
• Order and interpret labs, imaging, and diagnostic studies
• Prescribe medications and implement treatment plans
• Educate patients on disease prevention, treatment adherence, and lifestyle modification
• Conduct minor in-office procedures (e.g., wound care, injections, suturing, immunizations)
• Document all patient encounters accurately in the EHR
• Coordinate referrals and support continuity of care
• Collaborate with physicians, RNs, and allied staff
• Participate in quality improvement initiatives, case reviews, and care planning

Qualifications

• Active and unrestricted Arizona license as an NP or PA
• Graduate of an accredited NP or PA program
• Master?s degree in Health Sciences or related field
• For NPs:
  Approval by the Joint Board of Medical Examiners and Board of Nursing and active Arizona RN license
• Minimum 2 years of experience in outpatient family medicine or primary care
• EHR experience preferred
