Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations, visiting patients in their homes and coordinating care with health plans.
You will use an iPad with a streamlined clinical workflow, review medications and history, perform a physical exam, and conduct planned diagnostic tests when ordered by the health plan. Travel locally and within licensed states is required.
