Position Overview

The Wellness Nurse (WN) is responsible for supervising the delivery of care and services to residents while ensuring compliance with all state regulations, professional standards of care, and company policies. Ongoing communication with residents, families, associates, healthcare providers, and consultants is essential to resident well-being.

Key Responsibilities

Provide daily supervision and leadership to non-licensed associates and designated personnel

Assign work duties and meet regularly with associates to support performance and care quality

Monitor resident conditions and demonstrate sound clinical decision-making

Provide resident care per physician orders and regulatory requirements

Accurately administer medications and oversee continuity of resident care

Respond promptly to changes in resident condition or special care needs

Perform first aid and emergency interventions as required

Ensure care delivery aligns with recognized nursing practices and company standards

Maintain clear communication with residents, families, providers, and interdisciplinary teams

Perform other duties as assigned (see full job description)

Required Qualifications

Must be 18 years of age or older

Licensed Practical Nurse (LPN) in good standing with the Kentucky state licensing agency

CPR/AED Certification required

Minimum of 1 year of LPN experience in a senior care setting

Ability to pass a criminal background screening

Must comply with the Company Drug-Free Workplace Policy

Preferred Qualifications

Experience with
computerized resident documentation systems

Strong leadership, communication, and organizational skills

Ability to work collaboratively in a resident-centered environment

The responsibility of the Wellness Nurse (WN) is to supervise delivery of care and services to residents.
