Licensed Practical Nurse (LPN) Case Manager

Hocking Valley Community Hospital (HVCH) is currently seeking a Licensed Practical Nurse (LPN) Case Manager to join our friendly, patient-centered clinical team for the following shifts:

• Case Management/Social Services, Full Time, Days, M-F
• Pay starts at $20.81/hr to $29.03/hr based on experience

• Graduate of an accredited program in nursing.
• Active State of Ohio Licensed Practical Nurse Nursing License.
• At least 5 years of nursing experience in an acute health care setting.
• Good interpersonal and written and verbal communication skills.
• Computer literacy.
