• Provide direct nursing care and services to residents according to individualized care plans.
• Administer medications in accordance with physician orders, state regulations, community policies, and nursing best practices.
• Perform resident assessments as required by state regulations and community policies.
• Monitor residents for changes in condition and promptly report concerns to the appropriate supervisor.
• Maintain accurate and timely documentation of resident care, observations, communications, and clinical services.
• Assist with developing, updating, and maintaining individualized care plans.
• Collaborate with residents, families, physicians, and interdisciplinary team members to ensure high-quality care.
• Promote resident dignity, independence, and quality of life.
• Perform other duties as assigned.

This position reports to the Director of Assisted Living and/or Director of Memory Care. The LPN is expected to work independently, exercise sound clinical judgment, and may assist with mentoring, directing, and supporting other care team members as needed.

Required Qualifications:

• Current Licensed Practical Nurse (LPN) license in the State of Ohio.
• Current CPR certification.
• High School Diploma or GED.
• Minimum of one (1) year of nursing experience preferred.
• Strong clinical assessment, documentation, and communication skills.
• Passion for serving seniors and providing exceptional resident care.

• Ability to sit, stand, walk, bend, stoop, kneel, crouch, climb stairs, and reach throughout the workday.
• Ability to lift up to 25 pounds independently.
• Ability to assist with transferring, repositioning, pushing, and pulling residents using appropriate equipment and assistance when needed.
