Schedule:

Monday – Friday 8:00 AM‑5:00 PM, occasional evenings and/or weekends possible

About the job:

The LPN is responsible for the effective management and provision of participant medications provided via the PACE medication department. The LPN Med Tech administers medications, implements primary care orders, and evaluates participant needs physically and/or by telephone; provides education to Senior Comm Unity Care participants and caregivers in regards to plan of care and medications; and communicates with participant’s primary care givers, physician, clinic staff and PACE Interdisciplinary Team.

Responsibilities
  • Process medication orders for all participants, ensuring complete, accurate, and legal prescriptions are received by the dispensing pharmacy.
  • Review notes from outside providers and identify and process medication orders with approval of PACE provider.
  • Maintain participant medication schedules through implementation of new medication orders, mid‑cycle changes, discontinuations, and reconciliations through transition process as prescribed by policy.
  • Perform medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitate any changes required.
  • Communicate with and between PACE providers, community‑based providers, and the pharmacy regarding medication orders, clarifications, and pharmacy recommendations.
  • Coordinate with local pharmacies to obtain urgent medications that are not available through on‑site dispensing, including transmission of medication order, demographics, and insurance billing information.
  • Co‑facilitate on‑site medication dispenses through the system provided through the pharmacy, and perform periodic inventory and maintenance of on‑site medication stock.
  • Assess and enter all medication orders for new enrollments, setting up pharmacy cycle, delivery system, assistive systems if needed, linking diagnosis information to medications, and verifying all information in writing with participant’s primary care provider.
  • Coordinate medications for disenrolling participants to provide needed medications for a period of twenty‑eight (28) days after leaving program.
  • Assist medication technicians in filling and/or quality control of alternative medication delivery systems (MRB, Med‑E‑Lert, etc.) in accordance with medication orders and organizational policies/procedures.
  • Coordinate delivery of medications from pharmacy to participant either through direct carrier shipping or delivery through or from PACE center, maintaining documentation of delivery.
Qualifications
  • Medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact.
  • A minimum of one year experience working with the frail or elderly population.
  • Graduate of a school of professional nursing; current Kentucky Practical Nurse (L.P.N.) license;
    Cardio Pulmonary Resuscitation (CPR) certification.
  • Valid driver’s license, proof of insurance, and means of transportation.
  • All required qualifications will be verified before start date.
