Pay Range: $40.55 - $48.35

The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant’s condition to primary caregivers, physician and care team.
QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. REQUIRED- Current licensure in the state of Kentucky as a Registered Nurse.
- Must have a valid driver’s license and have means of transportation, to assist participants in their homes or other settings.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Current CPR Certification.
- A minimum of one-year (1) experience in working with the frail or elderly population

- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
