DescriptionPosition Summary: Necco has an opportunity for a career as Independent Living Case Manager/Life Coach
. This role will work with the young adult in an effort to successfully transition him/her to adulthood and to be a productive member of society. Work will be conducted in an office, community, employment, and/or school setting. The position adds value by equipping young adults with the services, supports, and resources needed to achieve independence as an adult.

The candidate selected will be responsible for the following:
Placement/Intake/Assessment- Participate in the referral matching process for young adults in an independent setting
- Shop for and prepare apartments for intake, including obtaining leases, ordering and assembling furniture, shopping for basic necessities, and cleaning to make apartments presentable for clients
- Facilitate and develop each young adult’s Initial Treatment Plan and Comprehensive Treatment Plan
- Ensure completion of appropriate assessments (Biopsychosocial, Independent Living, Diagnostic, etc.)
- Obtain initial legal documentation prior to placement or service delivery
- Develop safety plans during intake
- Develop and facilitate each consumer’s Individualized Treatment Plan (ITP)
- Assist the young adult with implementation, tracking, and monitoring of the Comprehensive Treatment Plan
- Meet and exceed all state and licensure regulations regarding client contact on the caseload
- Serve as liaison between Necco, placement agencies, and community resource entities
- Deliver services across multiple environments, including in‑home and community‑based settings, and transport clients as needed
- Complete monthly apartment inspections and follow up on work orders
- Participate in service meetings with community support representatives, state workers, outpatient clinicians, school personnel, and other invested parties
- Establish relationships with school personnel to ensure young adults meet education goals/requirements
- Provide ongoing training and support to young adults related to independence
- Monitor documentation to meet all state, licensure, and accreditation requirements
- Maintain confidentiality as outlined in policy
- Participate in a weekly rotating “on‑call” schedule for advisory and support to young adults
- Document and report on‑call activities and follow up with the direct supervisor
- Manage crisis situations promptly, including on‑site or face‑to‑face intervention if necessary
- Report to the supervisor or designated authorities any instances of non‑compliance, police contact, or reports of abuse, neglect, or suicidal ideation
- Ensure all critical incidents are entered into the Electronic Health Record within established time frames
- Complete and submit Medicaid documentation as applicable
- Ensure accurate data entry into the Electronic Health Record per state rules and company protocol (contact notes, academic supports, medical information, etc.)
- Attend and participate in daily huddles, weekly tactical meetings, monthly strategic meetings, and related activities
- Perform duties and responsibilities assigned by the immediate supervisor

- Current LSW or LPC or bachelor’s degree in a human services‑related field with a minimum of 2 years’ experience…
