Peace Health Hospice in Florence, Oregon is seeking a RN - Hospice for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at Peace Health is $49.30 – $77.46. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
This position is represented by a collective bargaining agreement. There may be more than one opening on this posting.

Job SummaryResponsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.
What you will do- Perform initial evaluation of patient/home and apply admission criteria to determine if patient is a safe candidate for home health/hospice services.
- Provide skilled nursing care to patients and provide skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
- Perform physical assessments, develop, coordinate, modify, and manage written individualized plans of care, as ordered by the primary physician, to meet patient’s long and short-term medical goals and appropriate/timely home health outcomes.
- Develop and clinically manage the Home Health Aide’s care plan according to the patient’s evolving personal needs.
- Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
- Perform discharge planning toward patient-specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
- Provide accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and Agency guidelines.
- Assist with the plan and implementation of family Bereavement services for the following patient death.
- Accredited School of Nursing required:
Nursing - Bachelor's Degree preferred:
Bachelor of Science in Nursing
- 2 years required:
Acute clinical or home health/hospice nursing experience - 1 year required:
Home health experience preferred or will orient to point of competency - Required:
Demonstrated knowledge of and ability to apply age‑specific principles of growth and development and life stages to meet each patient’s needs
- Registered Nurse in the state of Oregon required
- Driver's License and proof of auto insurance required
- Basic Life Support upon hire required
- Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
- Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
- Push/Pull:
Frequently up to 45 lbs. force (i.e., medical carts). - Perform hands on CPR at least 20 minutes using 100‑125 lbs. of force (National Association of EMS Physicians and AHA); frequency should be seldom or occasionally.
- Ability to move around area with frequent sitting.
- Bending, stooping, squatting, reaching, kneeling frequently.
- Lifting (non‑patient) up to 20 lbs. occasionally and 5 lbs. frequently.
- Exposure to biohazard, body fluids, and airborne particles.
- Must be able to complete tasks in a noisy environment.
- Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
- Ability to communicate and exchange accurate information.
