Peace Health is seeking a RN
- Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at Peace Health is $50.78 – $79.78. Hiring Bonus and Relocation Bonus may be available!

Responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.

Essential Functions

  • Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
  • Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.
  • Develops and clinically manages the Home Health Aide's care plan according to the patient's evolving personal care needs.
  • Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
  • Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
  • Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.
  • Assist with the plan and implementation of family Bereavement services for the following patient death.
  • Performs other duties as assigned.

Qualifications

Education

  • Accredited School of Nursing

    Required:

    Nursing
  • Bachelor's Degree Preferred:
    Bachelor of Science in Nursing

Experience

  • 2 years

    Required:

    Acute clinical or home health/hospice nursing experience
  • 1 year

    Required:

    Home health experience preferred or will orient to point of competency
  • Required:

    Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs

Credentials

  • Required:

    Registered Nurse in state of practice
  • Required:

    Driver's License and proof of auto insurance
  • Required:

    Upon Hire Basic Life Support

Skills

  • Basic computer skills including MS Office applications (Required)
  • Well organized, flexible and able to function independently (Required)
  • Cooperative working skills, leadership skills, and interpersonal skills and ability to work effectively with a wide variety of patients, families and staff (Required)

Working Conditions

Lifting

  • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
  • Push/Pull:
    Frequently up to 45 lbs. force (i.e., WOW, medical carts).
  • Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
  • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Environmental Conditions

  • Exposure to biohazard, body fluids and airborne particles.
  • Must be able to complete tasks in a noisy environment.

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
