Hospice RN Nurse Case Manager

Location:

Myrtle Beach, SC;
Conway, SC;
Little River, SC;
Murrells Inlet, SC;
Loris, SC;
Pawleys Island, SC.

This is a full‑time, salary‑based role. You must have reliable transportation, as daily travel to patients’ homes, long‑term care facilities, and other sites is required.

Schedule
  • 12‑hour shifts: 7:00 AM–7:00 PM (Monday–Sunday, 7 days on, 7 days off)
  • 8‑hour shifts: 8:00 AM–5:00 PM (Monday–Friday)

Responsibilities / Duties
  • Facilitate receiving all medical records from the patient’s primary provider and specialists.
  • Review medical records.
  • Complete consents with patients.
  • Enroll patients in Care Management when eligible.
  • Initiate a Care Management Plan of Care for eligible patients.
  • Capture diagnoses at the highest specificity, ensuring acceptance of gaps.
  • Complete annual wellness visits (AWVs) for provider review.
  • Complete cognitive impairment screenings.
  • Complete social determinants of health (SDoH) assessments and/or screenings.
  • Complete advanced care planning (ACP) discussions with the patient under provider review.
  • Evaluate suitability for home health, hospice, palliative care, or consults with the specialty division.
  • Evaluate suitability for remote patient monitoring (RPM) devices, resources, or tools that may improve quality of life.
  • Communicate and coordinate care.
  • Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
  • Provide post‑discharge education.
  • Evaluate for adaptive equipment and durable medical equipment (DME).
  • Evaluate for a safe environment.
  • Assess acute conditions or follow‑up from previous visits.
  • Appropriately document and log Care Management activities, and keep care plans up to date.
  • Coordinate with the patient’s health‑care team, providers, physical and occupational therapists, home health or hospice representatives, and other care plan members.
  • Facilitate visits with appropriate provider or entity.
  • Facilitate telehealth visits for coordination of care, when necessary.
  • Coordinate with the hospice interdisciplinary team and other care plan members.
  • Participate in interdisciplinary group (IDG) meetings, when required.
Qualifications
  • Registered Nurse with a license in good standing.
  • A minimum of three (3) years of clinical experience, preferably in community settings.
  • Proven ability to communicate and collaborate effectively with interdisciplinary care teams, patients, and caregivers.
  • Strong written and verbal communication skills.
  • Basic computer knowledge.
  • Demonstrated leadership and interpersonal skills.
  • Ability to foster a cooperative work environment.
  • Team player capable of managing multiple responsibilities with sound judgment.
  • Flexibility to work variable hours and travel between sites; must hold a valid driver’s license, possess a reliable vehicle, and maintain insurance as required by state or organization.
