Your Health Organization is seeking a Registered Nurse to join our visiting nurse team serving the Sumter area. The RN Case Manager will visit patients in homes and facilities, coordinate with providers, and initiate telemedicine visits.
Travel to patient homes and care facilities is required on a daily basis; a valid driver’s license and reliable vehicle are necessary. The role is full time, 8-hour shifts, Monday to Friday (8am–5pm), with responsibilities across care management and coordination
