Care Manager II; Foster Care

About This Role

The Care Manager II is responsible for managing and coordinating care for Members with various health conditions, serving as the primary contact for the care team.

This role is ideal for experienced professionals who excel in assessment, communication, and critical thinking.

• Location: Florence, SC
• Department/Unit: Medical Management and Quality
• Schedule: Full-time

• Support Members during transitions of care through assessment and coordination.
• Review medication lists and educate Members on pharmacy needs.
• Evaluate, monitor, and update care plans based on Member progress.
• Establish contact points to collaborate with community and health teams.
• Maintain accurate documentation of Member interactions in care management platforms.
• Monitor utilization and coordinate services with payer sources.
• Develop knowledge of care management programs and regulatory requirements.
• Identify cases for care management rounds and follow up with providers.
• Support a positive workplace environment and collaborate with diverse populations.
• Conduct face-to-face visits for education and assessment as needed.

• Bachelor of Science in Nursing (BSN) required.

• Minimum of 3 years of professional experience in a related field required.
• At least 3 years of case management experience preferred.
• Prior experience in the Foster Care System or related departments preferred.

• Current and unrestricted Registered Nurse (RN) license in South Carolina required.
